What is Chronic Exertional Compartment Syndrome (CECS)?

Chronic Exertional Compartment Syndrome (CECS) is a condition that causes pain and tightness in the muscles during exercise. It occurs when pressure builds up within a muscle compartment, restricting blood flow and impairing normal muscle function. This can lead to discomfort, cramping, and, in severe cases, long-term exercise intolerance.

When is Compartment Pressure Testing Recommended?

Compartment pressure testing is a minimally invasive diagnostic procedure used to confirm CECS in individuals experiencing:

  • Pain, tightness, or cramping in the muscles during physical activity
  • Symptoms that consistently resolve with rest but return with exercise
  • Numbness, tingling, or weakness in the affected area
  • Swelling or a feeling of fullness in the muscle compartment

How Does the Test Work?

  • Initial Assessment: Our doctors conduct a thorough clinical evaluation to determine whether compartment pressure testing is appropriate.
  • Baseline Measurement: A specialized pressure-sensing needle is inserted into the affected muscle compartment to measure resting pressure.
  • Exercise Protocol: The patient performs controlled physical activity (generally treadmill running)  to reproduce symptoms.
  • Post-Exercise Measurement: Immediately after exercise, the pressure is measured again to assess changes in compartment pressure.
  • Diagnosis & Treatment Planning: If pressure levels remain abnormally high, CECS is diagnosed, and treatment options are discussed.

A detailed medical history, (and) physical examination and ordering of appropriate investigations such as XR, MRI or dynamic duplex ultrasound to rule out other causes of exertional leg pain.

 In the appropriate clinical situation (eg. Diagnosis still unclear and/or attempt to rehabilitate unsuccessful and patient considering surgical management) utilising compartment pressure testing can help to obtain definitive diagnostic data.

Based on results, we provide tailored recommendations, which may include physical therapy, activity modification, or referral for surgical evaluation in severe cases.
